Social Worker - Community Mental Health Team (CMHT)

Location: Dorchester and Bridport, Dorset
Pay Rate: £35(Umbrella)
Hours: 37 hours per week hybrid working
Contract Type: Interim 6 months

This is an excellent opportunity for a Qualified Social Worker with adult mental health experience to support individuals with complex needs while working within a supportive multidisciplinary team. The successful candidate will undertake Care Act duties and will be required to carry out regular community visits as part of their caseload responsibilities.

Key Responsibilities

Complete Care Act assessments, support plans and reviews.

Undertake Section 117 assessments and reviews.

Assess and support adults with complex mental health needs.

Manage safeguarding, risk assessments and complex casework.

Work on cases involving MAPPA, MARMS, DoLS and Court of Protection.

Collaborate with CPNs, Psychiatrists, Psychologists and partner agencies.

Undertake Care Act duties and conduct community and home visits across Dorset.

Maintain accurate case records and statutory documentation.

Requirements

Qualified Social Worker with Social Work England registration.

Previous experience within Community Mental Health, Adult Social Care or CMHT services.

Strong knowledge of the Care Act, Mental Health Act and Mental Capacity Act.

Experience managing complex cases and risk.

Excellent assessment, report-writing and communication skills.

Ability and willingness to undertake community visits.
